JUAN SUMULONG HIGH SCHOOL ENROLLMENT PROCEDURES FOR SY 2021-2022

You may enroll either ONLINE of DROP OFF the ACCOMPLISHED ENROLLMENT FORM to DESIGNATED DROP OFF SITES. The steps below will Guide you on how it will be done.

STEP 1: DOWNLOAD the Basic Enrollment Form HERE

Printed Enrollment Form can be obtained and returned after completion from the school through the EARLY REGISTRATION COMMITTEE or Security Guard on duty  from Monday - Friday, 8:00 A.M. - 5:00 P.M. starting May 10, - June 9, 2023.

STEP 2. FILL UP and COMPLETE the INFORMATION needed. 

STEP 3. RETURN the ACCOMPLISHED LEARNER ENROLLMENT FORM to EARLY REGISTRATION COMMITTEE or Security Guard on duty  from Monday - Friday, 8:00 A.M. - 5:00 P.M. starting May 10, - June 9, 2023.

STEP 4: CONFIRM to designated grade level coordinators if they received your LEARNER ENROLLMENT FORM by calling them thru phone, cellphone, messenger or email.  Feel free to ask and take note of the important instructions they will give when you call.
